08-form-fields error adding block

Hi,

I’m getting an error adding the form field block to a post. “This block has encountered an error and cannot be previewed.”

All the other blocks work.

I’m running WP 4.9.4 with the Gutenberg plugin 2.5.0 and this commit of the block examples plugin. I note your commit messages ref V2 but the version in package.json is still 1.0.0, tut tut;-)
